Patriots Colony also has a full-time social worker who counsels family members and residents during transitional periods or health setbacks. Receive the Right Level of Care Needed At Patriots Colony your wellness is our priority. A health assessment is made prior to a resident’s arrival. This evaluation determines whether the new resident will reside in The Berkeley, Springhouse, or Convalescent Center. A health assessment also helps us determine if the resident is in need of any on-site services, such as physical, speech, or occupational therapy. Our goal is always to help our residents retain the highest level of independence possible. With such attentiveness to residents’ health status, we achieve this.
